Shenzhen Forward Video Co Ltd, DUTV005 v1.0A. This appears to be based on the Intel 9500 Reference Design (and given the USB id, it's a bit of a give-away).

 

Chipset: Intel CE6230

Tuner: Maxlinear MXL5003S

 

 Note: The Intel CE6230 was acquired from Zarlink. It is unclear what the Zarlink part number from the cross-reference table (TODO: Is it ZL10352 (MT352)?)
